📋 Tanggung Jawab Pekerjaan
- Assess and evaluate patient needs using clinical social work assessments and tools such as the Social Work Clinical Judgment Scale (SW-CJS).
- Develop and implement personalized care plans in collaboration with healthcare teams.
- Provide emotional support and counseling to patients and their families.
- Coordinate care with various healthcare providers, including doctors, nurses, and therapists.
- Monitor patient progress and adjust care plans as needed.
- Participate in team meetings and contribute to continuous quality improvement initiatives.

🎓 Kualifikasi & Syarat
- Bachelor’s degree in Social Work or a related field.
- Licensed Medical Social Worker (LMSW) certification.
- Minimum of 3 years of experience in medical social work.
- Strong knowledge of healthcare systems and patient care coordination.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in English, both written and spoken.
- Familiarity with electronic health records (EHR) systems such as Epic or Cerner.
